Assessment, active intervention, early progress and transition planning.
Treatment, behavior change, multidisciplinary coordination and maintenance.
Long-term maintenance, monitoring, independence and continuing-care planning.
Initial physiotherapy assessment, individualized rehabilitation plan, clinically indicated treatment sessions, therapeutic exercise, home-exercise plan, reassessment, functional outcomes, and transition planning.
The physiotherapist determines clinical suitability, frequency, progression, precautions, and discharge based on assessment and functional response.
Therapy House programs are individualized following assessment. Services, frequency, professional involvement, and level of care depend on clinical need, professional scope, service availability, and applicable requirements. Emergency or higher-level needs may require referral or transfer to an appropriate hospital or specialist service.
Start with Therapy House Central Intake. We can help identify the appropriate starting point based on your concerns and service needs — you do not need to diagnose yourself.
