90-Day Medical Recovery Bundle

Assessment, active intervention, early progress and transition planning.

  • Phase 1 — Medical Assessmentphysician consultation; medical history/examination; medication reconciliation; withdrawal-risk evaluation where relevant; vital signs; appropriate investigations/referrals; level-of-care decision.
  • Phase 2 — Stabilization & Monitoringphysician/nursing follow-up according to clinical need; medication monitoring within scope; laboratory/hospital referral when indicated; coordination with addiction/behavioral-health treatment.
  • Phase 3 — Recovery Health Planmedical progress review; chronic-health-risk identification; medication/adherence education; lifestyle recommendations; nutrition/physiotherapy referral when appropriate; final 90-day review.

6-Month Medical Recovery & Wellness Bundle

Treatment, behavior change, multidisciplinary coordination and maintenance.

  • Initial medical-recovery pathway plus scheduled physician reviews, nursing monitoring when indicated, medication review, relevant laboratory follow-up, chronic-condition coordination, recovery-health education, and multidisciplinary reviews.
  • Clinical emphasis progresses from acute medical recovery → stabilization → preventive health → self-management.
Continuing Care & Wellness

1-Year Recovery Health Management Bundle

Long-term maintenance, monitoring, independence and continuing-care planning.

  • Quarter 1 — Assessment and stabilization.
  • Quarter 2 — Medical recovery and risk-factor management.
  • Quarter 3 — Health maintenance and prevention.
  • Quarter 4 — Long-term health-management plan and annual medical review.

Medical services are individualized. Enrollment does not guarantee eligibility for outpatient withdrawal management and does not replace emergency, hospital, or specialist care.

Therapy House programs are individualized following assessment. Services, frequency, professional involvement, and level of care depend on clinical need, professional scope, service availability, and applicable requirements. Emergency or higher-level needs may require referral or transfer to an appropriate hospital or specialist service.
