Chronic Pain & Rehabilitation Program

Function-centered rehabilitation with physiotherapy and appropriate behavioral, medical and nutrition support.

Available

The Chronic Pain & Rehabilitation Program is designed for appropriate patients whose persistent pain affects movement, activity, confidence or daily functioning. The program emphasizes functional recovery and self-management rather than promising complete pain elimination. Physiotherapy is the rehabilitation anchor, with behavioral pain-management support and medical or nutrition input added when clinically appropriate.

Who may benefit

  • Adults with persistent musculoskeletal pain appropriate for outpatient rehabilitation
  • Patients whose pain limits mobility, activity or daily function
  • People with fear/avoidance of movement
  • Patients who may benefit from coordinated physical and behavioral approaches
  • Patients whose pain-related needs also involve medical or metabolic factors

Scope & safety

Persistent pain can have multiple causes. New or concerning symptoms may require medical investigation. The program should not guarantee complete pain elimination or replace necessary specialist care.

Key disciplines

  • Physiotherapy & Wellness
  • Behavioral Health
  • Medical review (when indicated)
  • Nutrition (when relevant)

Not every patient receives every listed discipline; the mix is decided after assessment.
